HOW TO SUBMIT YOUR ORDER You can submit your new order as follows: • Create/Login to your online account at ChampionSports.com • Email a copy of the completed form (or type the order details) in an email with subject line “Sports Inc Order” and send it to orders@championsports.com • Call us over the phone at 888.980.1200 • Print, fill-in and fax the form to 800.700.2934

FREIGHT CHARGES: • Invoices not paid by the due date will forfeit any partial or free freight allowances. NO EXCEPTIONS. FREIGHT ACCESSORIAL CHARGES: • Champion Sports is not responsible for inside delivery, delivery by appointment, lift gate delivery or residential delivery charges. • These charges will be incurred by the consignee. SHIPMENT INSPECTION: • When shipment is received, inspect cartons and carton count before signing delivery receipt. • Minimum order $1,000 F.O.B. Marlboro, NJ. Net 90 days. • $2,000 Order 100% FFA. Net 90 days. • $1,000 Order 25% FFA. Net 90 days. • In-season FFA Program Requirements: $2,000 or more booking order qualifies you for FFA on all $500 fill-in orders placed in season. Net 30 days. • Book any of the above programs and receive program pricing for fill-in orders. • Indicated pages in each section correspond to the Champion Sports 2019–2020 catalog.
